Received fresh, labeled with the patient's name, number, and “uterus, cervix, tubes, ovaries,” is a hysterectomy and bilateral salpingo-oophorectomy specimen consisting of uterus (8.0x4.5x4.0 cm), right ovary (4.2x3.5x3.0), right fallopian tube (5.5 cm in length and 0.5 to 0.9 cm in thickness), left ovary (3.0x1.8x0.6 cm) and left fallopian tube (it has been previously resected and submitted for research, and the remaining fallopian tube measures 3.0 x 0.5 cm). The external surface of the uterus is pink, focally congested and smooth; it is inked blue anteriorly and black posteriorly. The external os is 1.0 cm in largest diameter and the ectocervix is pink and soft. Upon opening the uterus the endocervical canal is 3.0x0.4 cm, pink tan, focally congested and otherwise unremarkable. The endometrial cavity is 3.5x2.0 cm, red-tan, friable, and there is necrotic tissue present which is loosely attached to the endometrium. The myometrium is pink to tan, congested and ranges 0.6 to 1.2 cm in thickness with a small leiomyoma measuring 0.5cm. No definite lesions are identified in the endometrial cavity, other than grossly necrotic, hemorrhagic, and friable tissue consistent with recent history of endometrial ablation. The right ovary has pink to red, cystic surface. Upon sectioning it reveals yellow-tan mucinous material with hair and cystic area at one pole. Upon removing the mucin the cyst wall is 0.1 to 0.2 cm in thickness, and the interior cyst surface is pink to red with a 0.5x0.5 cm soft projection present focally. The remaining ovarian parenchyma reveals physiological changes. The right fallopian tube is focally congested and otherwise unremarkable. The left ovarian surface is pink to red, lobulated and irregular. Upon sectioning the ovarian parenchyma reveals pink-tan to yellow cut surface with physiological changes. The remnant of the left fallopian tube is grossly unremarkable. The specimen is representatively sectioned and a portion of the friable tissue from the endometrial cavity and myometrium are submitted for frozen sections

62/M SUPRAGLOTTIC CA

Received fresh for frozen section, labeled with the patient's name, number, and “left supraglottic larynx,” is a left hemilaryngectomy specimen measuring 6x2.4x1.8 cm overall, containing a portion of epiglottis, false vocal cord, and ventricle. There is a 1x1.5 cm area, with 0.2cm in elevation, of white pink, granularity, likely representing tum or, residing within the false vocal cord, and possibly extending into the ventricle and true vocal cord. The tumor lies closest to the inferior resection margin about 0.2 cm, the posterior margin at approximately 0.3 cm, and is 0.5 cm from the anterior margin. The mass does not cross the midline, and lies 1.3 cm from the left aryepiglottic fold. The mass appears restricted to the mucosa and soft tissue adjacent to the mucosa, and does not appear to infiltrate the thyroid cartilage. The specimen has been inked by the surgeon prior to arrival in the following manner: black = inferior margin, green = posterior (assumed to correspond to the supraglottic surgical margin, adjacent to the thyroid cartilage), and yellow = anterior. The closet surgical margin is submitted for frozen section.

The ectocervical mucosa is unremarkabled. The endocervical mucosa appears unremarkable. On sectioning, there are one large submucosal mass measuring 15x12x12 cm in size. The cut surfaces appear multinodular, whorled, bulging, tan-white and firm with myxoid change. No necrosis is noted. There are several small nodules adjacent to a large mass measuring about 0.5 to 1.0 cm in diameter. The uninvolved myometrium is 3 cm in thickness. The endometrial cavity is distorted by the nodular mass. The endometrium is 0.2 cm in thickness and is otherwise unremarkable. No polyps noted

There is a flat tumor growing on the anterior endometrium (1.2x1.2x0.4cm). The tumor myometrial invasion. The myometrium measures 2.6 cm in maximum thickness and contains seven leiomyomas (ranging from 0.3 to 1.1 cm in greatest dimension) with white-yellow trabecular appearance without areas of hemorrhages or necrosis. The external surface is pink-tan. There are two areas of papillary growth (1.1x0.9x0.8 cm and 1.5x0.9x0.6 cm) of pink tan tumor on the serosal surface. The fimbriated end of the left fallopian tube is absent. The remaining portion of the fallopian tube is unremarkable. The cystically dilated has pink-tan smooth and glistening surface. Upon sectioning the left ovary is composed of multilocular cysts with a dominant cyst (5.2 cm in diameter). The cysts are filled with clear yellow fluid and have smooth and glistening internal lining. There is a focal area of solid growth (1.1x0.9x0.6 cm) located within the ovarian stroma. No residual uninvolved ovarian tissue is identified. The right ovary has white-yellow smooth lobulated surface. Upon sectioning the right ovary is composed of white-yellow sclerotic parenchyma with a single cyst (0.6 cm in diameter) filled with clear yellow cystic fluid.
