Discussion: HSIL is usually a lesion of smaller, less mature squamous cells. Nuclear enlargement is generally in the same range as in LSILs, but the nuclear to cytoplasmic ratio is higher because the cells are smaller. Large cell size is a feature of LSIL; prominent nucleolus and keratinization are usually seen in squamous cell carcinoma, rather than classic HSIL.
