Question No. 20. The Answer is: (A)  Coccidioides immitis.

Discussion: Coccidioides immitis form round spherules about 20-35 mm in diameter and endospores are present in the mature form as illustrated in this GMS stain (left panel). The spherules are large and can be recognized with just hematoxylin-eosin stain in some cases (right panel). Aspergillus species form hyphae. Cryptococcus neoformans, Histoplasma capsulatum, and Blastomyces dermatidis form yeast in tissue and are morphologically different from that of Coccidiodes immitis by the lack of endospores containing spherules.
