Discussion: In the low-magnification photo of this adenohypophysis, there are a few enlarged cells within a sea of normally sized cells. Storage diseases tend to affect most cells in the same tissue. This is slightly unusual. In the high magnification picture, some of the cells have huge nucleoli which are not a usual feature of storage disease. These cells do not appear like ova of a parasitic infection as there are no refractile cuticles. They lack the dotty cytoplasm of toxoplasma. Except for the huge nucleoli, the cell morphology is compatible with an amoebic organism. However, there are very few inflammatory cells and practically no neutrophils that are typically associated with amoebic infection. In reality, this is a case of cytomegalovirus (CMV) adenohypophysitis occur in an HIV patient.
